Various forms may be sent home during the school year. These may include:
1. Health history-this helps determine if there is a need for an individualized health plan for your child.
2. Individualized health plan-this is a plan of care with instructions on how to care for your child and his/her diagnosis. This is especially helpful if I am not in the building.
3. Asthma form-this alerts the school to the fact that your child has asthma, uses an inhaler, and what triggers the symptoms.
4. Food allergy- this helps us know what food(s) your child is allergic to. We can then alert the classroom teacher and kitchen.
5. Allergy-this lets us know what medications or other allergies your child has and how serious they can be.
6. Behavior form-this form is used for children who are on medication for ADHD. The teachers fill it out so I can see if the medication is effective. This is helpful for giving parents and physician information, especially if a medication or dose is changed.
7. A release may be signed if you have been offered a health plan and you don’t feel it is necessary for your child.
8. Letters will be sent home if we need documentation from you, such as vaccination dates or a dental form.
9. If your child needs to take prescription medication at school, we will need a form signed by both parent and physician.
